Post-Secondary Transition

What is Post-Secondary Transition?

Post-Secondary Transition is the vital step that every student with a disability must take as they leave high school and move on to further education, employment and independent living. Early planning for this step into adulthood is so essential that the transition process is a key element documented in each student's Individualized Education Program (IEP).

The Ohio Department of Education & Workforce provides information on post-secondary transition and workforce development for students with disabilities on their website. This site contains many resources for those interested in learning more about transition and workforce development. They have also developed a Secondary Transition Roadmap for families of students with disabilities.

Region 10 State Support Team is committed to assisting districts, students and their families with the secondary transition process. Please reach out to us with questions or if you need support.
